Forms and Policies
If you do not want to spend time filling out the History Form in the office, you can do it at your own time. Simply, print out the forms, fill them out and bring them to your consultation appointment.
Appointment Policy:
We always try to make our schedule run as smooth as possible, efficient and on time. Because your appointment has been reserved specifically for you, we greatly rely on your cooperation to let us know of any changes in your busy life schedule. We would greatly appreciate a 24 hour notice if you need to reschedule or cancel your appointment.
Emergency Policy:
If you or your child has any injuries to the mouth or teeth, please contact your general dentist immediately. If you have any broken brackets or damaged orthodontic appliances please call our office. An emergency appointment can be scheduled if you or your child is in pain from broken appliances.
Dental Care Policy:
It is extremely important to continue to visit your dentist during orthodontic treatment. We recommend a thorough examination and cleaning every 6 months.
